Cost of Legal Name Change in Canada

An adult`s name change request is used to change your name for reasons other than warlike. This type of app allows you to change all your names, as opposed to your last name. This service is subject to a fee. This type of name change changes the name on your Ontario birth certificate. The form is available online at ontario.ca. You can also request it by calling the toll-free number 1-800-461-2156 or, in the Greater Toronto Area, 416-325-8305. This service is subject to a fee. All application forms for changes of name, fees and supporting documents must then be submitted by mail or in person to the Office of the Secretary-General. You can change your name if you have a criminal record. The fingerprint matches your new name with your criminal record.
